Surgical oncology

This area covers lumps under the skin — in the thigh, the arm or the abdomen — moles that change and wounds that do not heal. Most are benign: a lipoma, a cyst. Sarcomas are uncommon, and with them the first surgery is what most determines the outcome.

What it is

These are the most frequent cases.

Skin and soft-tissue tumors arise in the skin or in muscle, fat, tendons and vessels. They can be benign or malignant; malignant soft-tissue tumors are called sarcomas. Sun and tanning beds (for skin), age, family history, certain chemicals and a weakened immune system add risk.

  1. LipomaA soft lump of fat under the skin. It is benign, and it is removed if it grows, causes discomfort or there is any doubt.
  2. LiposarcomaThe malignant one of fat. More frequent in the thigh, behind the knee and deep in the abdomen; between 50 and 65.
  3. LeiomyosarcomaOf smooth muscle. Usually starts in the abdomen or uterus, also in arms and legs.
  4. Dermatofibrosarcoma protuberansGrows slowly under the skin of the trunk or limbs. It rarely spreads, but must be removed with a margin.
  5. RhabdomyosarcomaOf skeletal muscle. It is uncommon and mostly affects children.
Signs and diagnosis

What is studied, and how the diagnosis is reached.

Signs that are studied

  • A lump under the skin that grows, especially if larger than five centimeters or deep
  • A new mole or one that changes shape, color or size
  • A sore or wound on the skin that does not close
  • Worsening abdominal pain, when the tumor is inside
  • Spots under or around the nails

How it is diagnosed

  • Examination of the skin and the lump
  • Ultrasound or MRI to see size, depth and relation to muscle and vessels
  • Biopsy of the suspicious lesion: the only one that confirms
  • CT to rule out spread once a sarcoma is confirmed
What the surgical oncologist does

What the surgical oncologist does, and what happens afterwards.

  1. 01Surgery with a marginIn sarcomas the tumor is removed whole, with a margin of healthy tissue around it. It is what most reduces the chance of recurrence, and that is why the first surgery is what most determines the outcome.
  2. 02Radiotherapy and chemotherapyDepending on type and size, before or after surgery, coordinated with medical oncology.
  3. 03Follow-upImaging check-ups for years: sarcomas can recur years later.

Frequently asked questions

The most frequent questions about this surgery.

  • Most likely it is a lipoma, which is benign. A lump that grows, is larger than five centimeters, hurts or lies deep under the muscle is studied with imaging before any intervention.

  • Small, superficial ones, yes, under local anesthesia. Large or deep ones are studied with imaging first.

  • Because if it were a sarcoma, removing it without a margin and without a prior biopsy complicates treatment later. First we determine what it is and then, with that information, it is removed with the appropriate margin.
