Surgical oncology

This area covers the esophagus, the stomach, the liver and bile ducts, the pancreas, the colon and the rectum. Colon cancer is among the most common and also among the most preventable: it starts as a polyp that can be removed years before it becomes cancer.

What it is

These are the most frequent cases.

Digestive tumors arise in the lining of the digestive tract or in the glands alongside it. Many start as benign polyps that transform over time. Age (most after 55), a diet high in red and processed meat, tobacco, family history and, in the stomach, Helicobacter pylori infection all weigh in.

  1. Colorectal cancerColon and rectum. It usually starts as a polyp; colonoscopy finds it and removes it before it becomes cancer.
  2. Stomach cancerIn the gastric lining. It produces few symptoms at first, which is why it is found late unless actively looked for.
  3. Pancreatic cancerIn the gland behind the stomach. Painless jaundice and weight loss are its signals.
  4. Liver and bile ductsHepatocellular carcinoma and cholangiocarcinoma. More frequent with prior hepatitis or cirrhosis.
  5. GallbladderUncommon, and often found when operating on a gallbladder for stones.
Signs and diagnosis

What is studied, and how the diagnosis is reached.

Signs that are studied

  • Blood in the stool or a change in bowel habit lasting weeks
  • Unexplained weight or appetite loss
  • Persistent abdominal pain or bloating
  • Nausea, vomiting or difficulty swallowing
  • Yellow skin and eyes (jaundice)

How it is diagnosed

  • Examination and labs: anemia is sometimes the first clue
  • Endoscopy or colonoscopy, which sees the tumor and takes the biopsy in the same act
  • CT or MRI to see the extent
  • Biopsy: the only one that confirms
What the surgical oncologist does

What the surgical oncologist does, and what happens afterwards.

  1. 01SurgeryIt consists of removing the affected segment together with its lymph nodes. In the colon, it can often be performed laparoscopically, with less pain and earlier discharge.
  2. 02ChemotherapyBefore or after surgery, depending on stage, coordinated with medical oncology. In the rectum, radiotherapy is added.
  3. 03AfterwardsIt requires several days in hospital. If the stomach was operated on, small and frequent meals help, and follow-up is carried out with colonoscopy and imaging.

Frequently asked questions

The most frequent questions about this surgery.

  • From age 45 if there is no family history; earlier if a first-degree relative had colon cancer or polyps. It is the test that prevents, not only the one that diagnoses.

  • It is almost always due to hemorrhoids or a fissure. But if it repeats, lasts weeks or comes with a change in bowel habit or weight loss, it should be studied with a colonoscopy.

  • In the colon, often yes: small incisions, less pain and faster recovery. It depends on the tumor and the patient; it is decided with the studies.
