Surgical oncology

This area covers the lung, the esophagus, the mediastinum and the chest wall: the ribs, the sternum and the tissues that cover them. A lump on the ribs or under the sternum should be evaluated calmly: many turn out to be benign, but all of them are studied.

What it is

These are the most frequent cases.

Inside the chest, tumors can arise in the lung, esophagus, trachea, thymus or lymph nodes; in the wall, in bone, cartilage or soft tissue. Tobacco is the dominant risk factor for lung cancer (about 80 % of cases), and secondhand smoke counts too. Asbestos, certain chemicals and family history also add risk.

  1. Chest wall lumpLipomas, cysts, cartilage inflammation (costochondritis) and, less often, bone or soft-tissue tumors. Studied with imaging and, if needed, biopsy.
  2. Lung cancerThe most common in the chest. Small cell or non-small cell; treatment differs in each case.
  3. Esophageal cancerSquamous or adenocarcinoma. Progressive difficulty swallowing is its most characteristic symptom.
  4. Mediastinal tumorsThymomas and lymphomas, in the center of the chest. Many thymomas are benign.
  5. MesotheliomaIn the pleura, the membrane lining the lung. Linked to asbestos exposure.
Signs and diagnosis

What is studied, and how the diagnosis is reached.

Signs that are studied

  • A lump on the chest wall, on the ribs or under the sternum
  • Chest pain that worsens with deep breathing or coughing
  • Persistent cough, hoarseness or shortness of breath
  • Difficulty or pain swallowing
  • Unexplained weight or appetite loss

How it is diagnosed

  • Chest X-ray: the first test
  • CT scan, which shows the exact size and location
  • Image- or endoscopy-guided biopsy: the only one that confirms
  • Labs and staging studies when the diagnosis calls for them

  • No. For many chest tumors the main treatment is chemotherapy or radiotherapy, and surgery is one part. It is decided jointly.

Before surgery

Before surgery, you can request a second opinion.

If you send us your studies, Dr. Jacobo reviews them and explains whether there is another option, or whether the first opinion was the right one.
